Skilled Nursing

At Hillcrest, the skilled nursing neighborhood in La Verne provides a welcoming, home-like setting where compassionate care and comfort come together. Woods Health Services, licensed by the state of California and certified by Medicare and Medi-Cal, includes 59 private and semi-private beds. Individualized care plans are designed in collaboration with doctors, family members and those receiving care, beginning at admission and updated regularly to reflect evolving needs. Professional nursing services, medication support, housekeeping, daily meals with snacks and activity programming are all part of the monthly fee, with additional offerings available for personal needs and preferences.
Rehabilitation
The Hillcrest rehabilitation program, operated through Woods Health Services, offers 24/7 care from registered and licensed vocational nurses and certified nursing assistants. Physical, occupational and speech therapy professionals work alongside each person to support strength, healing and well-being. A social service designee helps ease the transition into the health care setting.
Each day brings opportunities to engage in individual and group activities: crafts, games, socials and special events. Regular visits from the Hillcrest chaplain provide space for spiritual connection through private consultations, interfaith services, Bible classes and alternative recovery groups.
Memory Care
Memory care at Southwoods Lodge is designed with thoughtful attention to the unique needs of those living with Alzheimer’s and other forms of cognitive impairment. This close-knit neighborhood includes 24 rooms (six private and nine semi-private) each with a private bathroom. Personalized care plans are implemented with kindness and intention by specially trained team members. Participation in the rhythms of daily life is encouraged, from preparing and sharing meals in the country kitchen to enjoying time in the cozy living room with friends and family. Two secured gardens invite peaceful wandering, rest and connection with nature. Memory care at Hillcrest includes therapies that promote emotional well-being and engagement, such as art, music, pet interaction and exposure to natural light. Around-the-clock support, daily activities, meals, housekeeping and assistance with daily living are all part of the monthly rate, creating continuity, comfort and community.
NOTE: Memory care is reserved and prioritized for Hillcrest residents already on campus. However, occasionally, there are openings for non-residents. Call or send an email to check on possible availability in memory care.
